These terms and conditions (Terms) govern the Play Baseball Team Rebate Program (Program) and constitute an agreement between The Golf Warehouse, LLC, which operates the Play Baseball brand (Play Baseball), and each participating team. By participating in the Program, each participating team agrees to these Terms.

Eligibility: The Program is open exclusively to registered baseball and softball teams, with eligibility determined solely at the discretion of Play Baseball. Participants must provide a valid Employer Identification Number (EIN) or Taxpayer Identification Number (TIN) for the organization. Failure to provide this information may result in disqualification from the Program.

Enrollment: Teams must express their interest in joining the Program by sending an email to [email protected] with the following required information:

• Primary contact's name and email address
• Organization name
• Team name
• Requested 30-day promotion dates (the Promotion Period)

Play Baseball will confirm the Promotion Period via email. Only complete submissions will be considered; incomplete requests may be rejected.

Promo Code: Upon successful registration, teams will receive a unique promo code valid exclusively for the confirmed Promotion Period. Play Baseball reserves the right to modify or revoke promo codes at its sole discretion, including in cases of suspected misuse or fraud. Any changes will be communicated promptly.

Rebate Details: A 10% rebate will apply to all Qualifying Sales, which are defined as purchases made through www.playbaseball.com using the unique promo code during the confirmed Promotion Period, less any discounts, returns, or other adjustments. Play Baseball will verify and determine the total Qualifying Sales within (45) days following the end of the Promotion Period. Rebates exceeding USD $600 will require the issuance of IRS Form 1099 in compliance with tax regulations. The participating organization is solely responsible for reporting and paying any taxes associated with the rebate. Play Baseball reserves the right to deny or adjust rebates in cases of policy violations, suspected fraud, or other exclusions, such as returns.

Privacy Policy: Participants' personal information will be managed in accordance with the Play Baseball's Privacy Policy, located at https://www.playbaseball.com/ourcompany/privacypolicy.

Limitation of Liability: To the fullest extent permitted by law, Play Baseball shall not be liable for any indirect, incidental, special, or consequential damages arising out of or in connection with the Program. Play Baseball' total liability for any claims related to the Program shall be limited to the total rebate amount owed for Qualifying Sales.

Amendments and Termination: Play Baseball reserves the right to modify, suspend, or terminate the Program or these Terms at any time, with or without notice. Participants will be notified of material changes via email or other reasonable means. Continued participation after modifications constitutes acceptance of the updated Terms.
